The health assessment is a questionnaire you complete that provides you with a comprehensive view of your health. Following completion, you receive a personalized wellness summary to help you make, and meet, your health-related goals.
Completing the health assessment takes 15-20 minutes.
If you prefer to take the health assessment on paper, contact APS Healthcare at 1.866.272.5507 and they will send you the questionnaire.
Employees and spouses are able to complete the health assessment for a $25 incentive. The deadline for completion was November 30, 2011.
