Incentives for FY2013

You are eligible to receive a Take Charge! Live Well! incentive if you are enrolled in one of the state's health plans and complete eligible activities. You may also receive an incentive for your enrolled spouse's participation. Eligible employees and spouses can each earn up to $350 in reward cards by completing your Pathway to Health.

What do my incentive cards look like?

Do I have to complete the Well-Being Assessment and the physician form to be eligible for the coaching incentive?
Yes, members have to complete both the Well-Being Assessment and the physician form to qualify for the coaching incentive.  For details on how to complete your Well-Being Assessment, click on the Well-Being Assessment guide to your right.
Do I have to complete the Well-Being Plan and the online activities to be eligible for my online path incentive?
Yes. Both the Well-Being Plan and designated online activities (outlined in the Reward Center tab on the Healthways website) must be completed to earn the $200 online pathway incentive.
Will I receive a separate incentive for completing my Well-Being Plan?
No. You will receive the $200 incentive for completing the online path once both the Well-Being Plan and online activities have been completed.
Do I have to request my incentive?
No, your gift card reward will automatically be sent to you upon completion of an incentive-eligible activity. Your gift card will be sent to your current OAKS address.  You will receive a separate envelope and gift card for each incentive activity completed.
Do taxes apply to my incentive?
Yes. The taxes on the amount of your incentive will be added as income and taxed in your paycheck.
How much are the taxes?
The incentive card amounts are added to the taxable gross income on your check and a flat rate tax is applied since it is considered an award.  Flat rate tax runs approximately 25% federal and 3.5% state.
 
The incentives paid for the Take Charge! Live Well! program have always been considered taxable income. It may appear to be a higher tax this year as the incentive itself is not being paid in the paycheck,  however the tax rate has not changed.
How does my incentive appear on my paycheck?
Your incentive will appear as TCLW CC AW (Take Charge! Live Well! credit card award).
How will I receive my incentive?
Healthways will allow you to track your incentive participation and completion information on their online portal (go to www.ohio.gov/tclw). In addition, incentives will now be paid in the form of gift cards.  Please allow 7-10 weeks for delivery.
The Visa gift card will be sent to your home address listed in the State of Ohio OAKS system. In order to ensure receipt of your incentive, make sure you have your most updated address in the system. You can check this by going to myohio.gov.
When can I expect to receive my incentive?
Please allow 7-10 weeks for delivery following the completion of you Take Charge! Live Well! activity.
If you have not received your incentive, call Healthways at 1.866.556.2288.
If I need to contact Chase, what number do I call?
For checking balances, activity, or reporting a stolen card, call 1.877.669.9810.
For fraud cases call 1.877.669.9808.
If I think I accidentally threw my gift card away, what do I do?
If you think you accidentally threw your gift card away, call Healthways at 1.866.556.2288. A $5 reissuance fee may apply.
Do I receive one gift card for all of the Take Charge! Live Well! activities I complete?
No. You will receive a separate gift card for each Take Charge! Live Well! activity completion.
Will my spouse's incentive reward card come in my name or his/her name?
Your spouse's incentive reward card will come in his/her name in an envelope with the Healthways logo.
Is there an expiration date for the gift cards?
No, however, after six months of inactivity, there is a $5 per month reduction in card value.
Do I have to activate my card before I can use it?
Yes. You can activate your cards by going to mychasepaymentcard.com or by calling the Customer Service phone number on the back of your card.
How long do I have to activate my gift card?
You have 90 days to activate your gift card per the card’s terms and conditions.
Where can I use my gift card?
You can use your gift card anywhere that VISA is accepted.
How do I use my gift card at the store?
You will need to use your gift card like a credit card so no pin is required.
Who do I contact if I have questions about my gift card?
You may contact the customer service number listed on the back of your card or via the website at mychasepaymentcard.com. You can also write to P.O. Box 551667 Jacksonville, FL 32255.
