The Take Charge! Live Well! diabetes program provides you with:

Effective 7/1/2012, members will now be eligible for free diabetic supplies and medications without being engaged with a health coach if they meet certain clinical guidelines. The required guideline is that the eligible member has a Hemoglobin A1c test in the last 12 months. 
For members engaged with a health coach as of June 30 will continue to be eligible for free supplies and medications and will have a six month grace period to have the Hemoglobin A1c test.
Starting January 1, 2013, you will need to have had at least one Hemoglobin A1c test in the past 12 months to be eligible for free supplies and medications. Since it may take 90+ days from the date of the Hemoglobin A1c test for the results to be sent to Healthways, you should schedule your test no later than late September.
Certain values for this test are not required, only that the member has had the test. For those that meet the guidelines the supplies, durable medical equipment, and medication listed in the chart below are provided at no copay and no deductible.

Items are covered by your health plan when acquired via an in-network provider. Please verify coverage with your health plan prior to obtaining equipment.

Diabetic Supplies:
Items are covered by Catalyst RX through a retail store, specialty provider or a traditional mail-order. Prescriptions are needed in order to fill requests for these supplies at $0 copay.

Tapes & Prep Wipes
- Liquid adhesive and adhesive remover wipes
Alcohol prep wipes/pads
- Tape, adhesive patches, dressings

For members that have not met the clinical guidelines, they will not qualify for the free diabetic supplies and medications. They will have coverage for these items with copays and deductibles under the normal provisions of the State of Ohio benefits.
