What services does Healthways provide?
Healthways provides a well-being assessment – known as the Well-Being 5 survey – as well as health coaching, online tools and trackers, biometric screenings, physician form, and nurse line services.

The Nurse Advice Line is available 24 hours a day, 7 days a week.

Who is eligible to participate in the Take Charge! Live Well! program?
State employees and spouses enrolled in a State of Ohio health plan are eligible to participate in the program for incentives. Dependents other than spouses are eligible to participate in the asthma and diabetes coaching programs with no incentives by contacting Healthways.
What should I expect with with working with health coach?
You will have the opportunity to work with a health coach. You may already have been contacted by a Healthways coach. In addition, you can contact Healthways at
1-866-556-2288 and request to work with a health coach.
What is health coaching?
Working with a health coach provides personalized support to help lower your risks, manage your conditions, and change behaviors. A health coach can help you set and keep you on track to achieve your health goals. Each coaching program will be created and personalized on your unique needs and goals.
Is everyone eligible for health coaching?
Employees and spouses are be eligible for health coaching. Based on the results of your biometric screenings and your Well-Being 5 survey, a health coach may reach out to you to offer support to take the next steps to improve your health.
DIABETES MANAGEMENT PROGRAM
How is the Diabetes Management Program changing?
Members are eligible for free diabetic supplies and medications without being engaged with a health coach if they meet certain clinical guidelines. Certain values for each test are not required, only that the member has had the test. The required guideline is:
- Had Hemoglobin A1c test in last 12 months
If the criteria is met, members will be eligible for free diabetic supplies and medications without having to engage with a health coach. For members that have not met the clinical guidelines, they will not qualify for the free diabetic supplies and medications.

What is a Well-Being 5 survey?
The Well-Being 5 survey is a confidential questionnaire that assesses your physical, emotional and social health and how your lifestyle habits affect your overall well-being.
Once you complete the Well-Being 5 survey, you receive an online Well-Being Plan. The plan consists of:
- A personalized summary of your overall well-being that offers actionable steps you can take to improve well-being.
- Personalized recommendations and focus areas to keep you motivated and on track.
- Fitness, nutrition and stress management plans that promote healthy behaviors to help you reach your healthy best

What is a biometric screening?
A biometric screening is a private screening with a health professional to get a current view of your health. It is a quick blood test that checks your total cholesterol, LDL level, HDL level, triglycerides, blood glucose level, blood pressure and body mass index (BMI).
You can complete the screening either onsite (check ohio.gov/tclw
for dates and sites) or have your physician complete the Physician Form available on the Take Charge! Live Well!

How long do biometric screening appointments take?
Each biometric screening appointment takes approximately 15 minutes, although it is recommend that you arrive at least 10 minutes before your scheduled appointment to complete your paperwork.
If there is not a biometric screening event scheduled at my agency, how can I earn my reward?
During your annual physical you can have your physician complete the Physician Form.
What will happen to my information?
Healthways protects the confidentiality of your information in compliance with the Health Insurance Portability and Accountability Act (HIPAA) of 1996, which prohibits anyone from receiving your personal health information without your permission. Healthways may provide the State of Ohio with collective data about its population as a whole, but not any individual health information. Healthways does not sell your information and is not a telemarketing service.

Why should I participate?
Your well-being is important. With the right information, you can be better prepared to address any health risks that could possibly affect the quality of your life. Because some risk factors may have no symptoms, you could be at risk without even knowing it. And, with the right tools and personalized support, you can get help to take the little steps that keep you moving along the path to a better you.
Am I able to participate if I am already healthy?
There is always something to work on! Members that already consider themselves healthy can complete the Well-Being 5 survey, complete a Well-Being Plan, complete online activities, and complete a biometric screening. There is something for everyone in this program.
