Diabetes Management Program

The Take Charge! Live Well! diabetes program provides you with:

  • Free health coaching opportunities with regular outreach to you
  • Complimentary educational materials

Members are now eligible for free diabetic supplies and medications if they meet certain clinical guidelines. The required guideline is that the eligible member has a Hemoglobin A1c test within the past 12 months.

Certain values for this test are not required, only that the member has had the test. For those that meet the guidelines the supplies, durable medical equipment, and medication listed in the chart below are provided at no copay and no deductible.

If you have questions about the program and your eligibility, contact OptumRx at 866-854-8850.
